Checklist of requirements

1) Birth Certificate Application

2) Provide Hospital Documents (Afghan Birth Certificate, translated and confirmed by the Ministry of Foreign Affairs)

3) To be issued a birth certificate you will need to provide your Original Valid Afghan Passport OR Translated and confirmed Tazkera from the Ministry of Foreign Affairs Afghanistan.

If Valid AFGHAN passport or Translated Tazkera is not available, you must apply for a NEW Afghan Passport and complete the requested requirements.
